The documents below will assist in familiarizing your agency with the EVV program. Among the resources below are the EVV Service Code Listing which identifies which service codes are EVV mandated and which are optional and the Finalized Reason Code List.

All providers must complete the mandatory EVV training to request and receive their Welcome Kit. New providers are encouraged to complete the EVV training while they are completing the DSS enrollment process to prevent a delay in receipt of their Welcome Kit. Please follow the directions below to enroll in the Learning Management System (LMS), a web-based learning system that provides the training videos and documentation that must be reviewed prior to requesting and receiving the EVV Welcome Kit for your agency. Once you have completed all of the training videos, please contact Sandata Customer Care via email at ctcustomercare@sandata.com to request electronic delivery of your Welcome Kit. Delivery of the Welcome Kit may take up to two (2) weeks.
