To download a current Fee Schedule, do the following:

1. Review the End User License Agreements.  In order to access the fee schedule, you must accept the end user license agreements. Select either:
   • I Accept
   • I Do Not Accept
2. Select the desired Fee Schedule from the list on the panel.
3. Click on "PDF" link to view and print the footnoted fee schedule, if available or
4. Press and hold the CTRL key while clicking on the "CSV" link in order to download the fee schedule in a format that is compatible with Microsoft Excel.
1. Select "Save" to download a copy of the Fee Schedule to your local machine, or
2. Select "Open" to open the Fee Schedule in Microsoft Excel format in a new browser window.
5. The Fee Schedule lists all procedure codes for the selected provider type, including pricing modifiers if present, maximum fee, effective date, end date, short description, and indicators for prior authorization.
