NDC
Enter the 11 digit National Drug Code (NDC). You must enter 11 digits. An 11 digit NDC has a 5-digit labeler code, a 4-digit product code and a 2-digit package code. If any of these segments of the NDC is fewer digits, then add a leading zero to one of the segments. For example, if the 10 digit NDC is 1234-1234-12, then enter 11 digit NDC 01234-1234-12.
